Based on the survey, April 2014 in Gambiran Hospital Kediri stated that before facing the repatriation, mostly patients have unready category on the time going back home caused by unoptimality discharge planning program. The purpose of the research was to know the influence of discharge planning to the readiness of diabetes mellitus patient to go back home. The research used pre-experimental design with 7 respondents. The sample was some patients cared in Gambiran Hospital of Kediri City on June 2014. The sampling method used in this research was total sampling. Data collection technique was by the quesionaire. The identification of the various readiness levels to face the repatriation a patient of pre and post discharge planning was analyzed by using non-parametric wilcoxon statistic. The result of this research showed that before discharge planning, most of the respondents (57,1%) had score 30-35 which means lack of readiness. After discharge planning their score increase upto 42-48 in (57,1%) patient. This number means that they had a well readiness. The the statistic test result showed that ρ = 0,02 ( ρ value < 0,05 ). The conclusion from this research is that there is the influence of discharge planning to the readiness of diabetes mellitus patients to go back home. Therefore, the research hoped to give the advantage for every medical practitionaire in which they should do a discharge planning optimally for the patients’ readiness to go back home.
